We encourage all our clients to become active in decision making helping to shape the way in which their support is delivered. Following your initial contact, our team will arrange to carry out a no obligation, free cost initial assessment home visit at your convenient. This initial assessment enables us to get a better understanding for the services you require. The assessor will listen to you; complete an assessment form that gives a full picture of your requirements based on your needs, wishes, and preferences including personal care, health needs, mobility, hobbies, goal and aspiration. This information is then used to make your support plan, skills match with the carers and to inform the care support worker of your needs.
